Please only complete this form if you have confirmed a lesson time and day with me directly.

Congratulations! You have started the process for signing up for swimming lessons with me. I have taken steps to remove all of the paperwork from the business and the booking form is one of those pieces removed. I am replacing it with an online form to capture all of the relevant details.

By completing this form you agree that your details can be used whilst you are using my services to contact you in relation to your booking.
If you have booked 2:1 lessons please complete 1 form entry per swimmer.
If at any time you need to update any of the details provided you do not need to complete this form again, please email info@swimwithbecky.co.uk with the required changes.

Bookings:
Booking enquiries can be made online, over the phone or in person but places will only be confirmed once payment is received. A booking form will need to be completed fully and returned online.
Invoices &Payments:
All weekly lesson bookings are paid for in blocks of 3 lessons, and intensive lessons are paid for in full week blocks.
New bookings will have an invoice and link for the booking form and terms and conditions sent via email. For transparency my terms and conditions are available on my website.
Your place will only be confirmed once full payment has been made.
Spaces are held on a rolling basis and you will receive an invoice the week before your last lesson is due for the next block. Should you wish to stop lessons I do ask for at least 2 weeks notice prior to the current block ending.

Spaces are held on a rolling basis. This means you will receive an invoice before the last lesson of your current block, which will cover your next 3 week block. Should you wish to stop lessons I ask for atleast 2 weeks’ notice prior to your current block ending.
Payments are to be made by bank transfer only.

Should you wish to cancel your booking, or cannot attend for any reason no refund will be given.

Cancellations/Refunds:
I do not offer credits or refunds for general sickness, injury or holidays. For longer term illness or injury that can be supported with a doctors note please contact me directly to discuss the situation and I will try to accommodate you as best I can.
No refunds will be given for non-attendance.
Where lessons are cancelled by myself for any reason, where possible I will try to organise a catch up lesson that week or during that current block. If this is not possible then you will have this credited to your next invoice.
Class structure:
My lessons are designed to be 1 to 1 and 2 to 1 lessons. This is to achieve the best outcomes each lesson. 1:1 lessons are very effective but it is possible ‘to buddy up’ like friends or siblings. As long as the swimmers ability allows this to happen it is a very effective and a good way to share the cost of private lessons.
Parents/Carers/Guardians:
Parents/Carers/Guardians of anyone who is 18 years or younger need to remain in the onsite for the duration of the lesson for the safety and wellbeing of the swimmer.
When waiting in the changing hut or anywhere onsite at the private pool noise levels must be kept to a minimum and any other children in your care kept under strict control to avoid distraction to the lesson, any other users of the pool or neighbours.
Food and drink is not permitted on the poolside with the exception of a sealed bottle of water where required or where there is a medical requirement.
Pool Safety Rules:
These rules are in force both poolside at all times
-
No Running
-
No Jumping
-
No Diving
-
No Mobile Phones (Photos/video)
Lateness:
It is the responsibility of the adult bringing the child to the lesson or the swimmer themselves where over the age of 18 years to ensure they arrive in good time for the start of the lesson. I usually suggest arriving at the venue around 5 minutes before your lesson commences. If you are late to your lesson then you will only be entitled to the remainder of your booked session with no refund or credit.
Personal Belongings:
No responsibility is taken by myself for the loss or theft of personal belongings. If you do misplace any items please check with myself.
Dress Code:
All swimmers should wear appropriately fitted trunks/costumes.
Long hair should be tied back and swimming hats can be worn if you wish.
Goggles must be adjusted and fitted before entering the pool.
Standards of behaviour:
I aim to make all lessons as fun and productive as I can where swimmers are encouraged to learn new skills as well as build up on ones they already have. To help swimmers achieve this it is expected there are a high standard of listening and behaviour skills. Disruptive and/or dangerous behaviour will not be tolerated as this can affect the learning and safety of not only the individual but other users of the pool. If for any reason a swimmer is acting in a disruptive and/or dangerous way or persistently ignores instruction then a warning will be given to the swimmer. If and when one warning has been issued the swimmer will be asked to sit on the side for a short ‘cooling-off’ period. If the behaviour continues after the cooling off period I will ask the parent/guardian to have a final word with the swimmer – after this the persistent swimmer will be removed. If a swimmer has any learning or behaviour difficulties I must be made aware of these on the booking form and if they confirmed after completing the form I need to be informed in writing. This will allow me to take appropriate action and where applicable it may be necessary to discuss a discipline or learning strategy to benefit the swimmer.
In exceptional cases I do reserve the right to exclude a swimmer for part or the whole of their swimming lessons for the remainder of the half term, whether this has been paid for or not. If you have any questions about this please contact me at any time.
Medical Conditions:
It is essential that on the booking form any medical conditions or learning difficulties are declared so I can plan appropriate lessons. Any medical conditions coming to light once a swimmer is booked must also be notified in writing.
The pool:
As with all pools there may sometimes be times where maintenance is needed and I have no control over this. In the case of this causing disruption to lessons where they may be unable to run I will contact you as soon as I am aware via email, text and telephone call. Please ensure your personal details are kept up to date for this reason.

Where possible we will try to organise a catch up lesson that week or during that current block. If this is not possible then you will have this credited to your next invoice.
COVID-19/illness:
Although the guidelines with regards to covid-19 have now been dropped I ask that you do not attend lessons if you/the swimmer, any accompanying adults, anyone in your household or anyone in your bubble have symptoms or have tested positive for Covid-19. In addition to this if you have advised to self-isolate then please ensure you follow this.
If you arrive at a lesson and the swimmer or accompanying adult are showing symptoms of covid-19 (or any illness) I retain the right to cancel your lesson and request you leave the premises immediately. This is for your own wellbeing as well as mine. This also includes symptoms appearing during lessons where we will end the lesson and request you return home.
We ask that you help us to help keep all of our other customers and teachers safe, and allow us to continue delivering this lifesaving skill by following this guidance.
This also covers any contagious illness such as seasonal colds, flu and bugs.
No mobile phones are to be used on poolside with the exception of by Becky for emergencies.
By paying and attending lessons you are agreeing you have read, understood and agree to all of the terms.
